The Optimum programme: a structured pathway around the patient.
Set up at Médipôle Lyon-Villeurbanne, the Optimum programme is more than a remote monitoring tool. It is a complete care pathway built on three components.

The striking feature is the population: a median age of 85 years. That is well beyond the cohorts usually studied in remote patient monitoring, and it speaks directly to the question of accessibility in very old patients.

The alert volume is worth reading carefully: 387 alerts for 5,323 questionnaires, about 7% of signals. That is a workload a care team can sustain.
What this study does not show.
- There is no comparator group. The reduction compares the period after enrolment with the patients’ own prior situation. It cannot be attributed to remote follow-up alone.
- The programme is multi-component. Multidisciplinary assessment, laboratory follow-up and remote monitoring act together; the contribution of each cannot be isolated.
- These are preliminary results, from a single centre, presented at a congress.
- Remote monitoring replaces neither emergency care nor the medical relationship.
